To comply with radiation exposure regulations, submit an Authorization to Obtain Radiation Exposure History form if you have a previous or ongoing radiation exposure history with another institution. Only list institutions where you wore a personnel monitoring device. Fill out this form and fax it to Health Physics at (650) 723-0632.
