How to Use

This tool is designed to help you identify potential ergonomic risk factors at your computer workstation. If you identify a risk factor, inform your supervisor and create an action plan to control the risk.

Once you complete the form, click the “Download PDF” button in the top right corner to retain a copy for your files. A copy of the results will also be emailed to you.
