University faculty who need to buy precursor chemicals for research must submit this form to begin the request process. We will need a statement of intended use and possibly a SOP. See webpage for more information.

After submission of Form 5, it will be sent directly to the PI for certification. After the PI certifies the form, the form will be sent to EH&S for processing. Each submitter will receive an email detailing the progress of the submission, along with a copy of their submission. Please keep a look out for any emails from the SU Precursor Program with SECURE: in the subject line.

After processing the application, we will place the order with Spectrum Chemicals. Upon receipt of the shipment, we will notify the authorized researcher to schedule pick up and to transfer directly to the research lab. EH&S will charge the submitter’s PTA for the controlled substance.
