Fill out this form to order dosimetry or if you are uncertain if you need dosimetry. Established locations or groups in the clinic or research community that receive dosimeters have a dosimetry “contact.” This contact helps you get a dosimeter if you need it.

If you are a location contact for dosimetry with an established account, you can begin service requests with the Radiation Dosimetry Service Request web link. For help, contact the Health Physics dosimetry coordinator.

Note: If you have a previous or ongoing occupational radiation exposure history with another facility, submit an Authorization to Obtain Radiation Exposure History form, or select “No” to indicate no radiation exposure history. It is your responsibility to inform Health Physics of historical or concurrent radiation exposures from other facilities.
