How to Use

Seasonal influenza vaccine is available to employees with occupational exposure. This includes Department of Public Safety (DPS) and Stanford University Occupational Health Center (SUOHC) employees. The vaccine does not have to be available outside of the period designated by the Centers for Disease Control and Prevention (CDC).

Employees with occupational exposure who decline the seasonal influenza vaccine must sign this form.
