How to Use

This notification is intended to inform building occupants of potential renovation activities that may impact individuals. To minimize impacts on local building occupants, project managers must notify the building manager who will:

• Notify the building occupants about the project via this form or by similar means and,
• Distribute project notices to affected personnel via email and/or post

Project managers should inform building occupants at least five days before beginning the project.
