Fill out this form to transfer radioactive materials from one Controlled Radiation Authorization (CRA) to another that exceeds 10 x QLM value. Put the original document and one copy into your Radioisotope Journal and give the recipient two copies. Both CRAs must attach a copy of the form to their quarterly report. For transfers less than 10 times the QLM, record in your Radioisotope Log.
