How to Use

The Stanford University Occupational Health Center (SUOHC) offers vaccinations to its employees with occupational exposure. These vaccinations include those recommended by the California Department of Public Health, as listed in the table of Aerosol Transmissible Disease Vaccination Recommendations for Susceptible Health Care Workers.

Employees with occupational exposure who decline a recommended and offered vaccination must sign this Vaccination Declination Statement for each declined vaccine.
