How to Use

This Confined Space Evaluation Worksheet helps you identify whether or not a space is a confined space. Assign the space one of these three classfications:

• Not a confined space
• Non-permit required confined space
• Permit-required confined space

You can use the companion guide to determine whether the space requires a permit and identify hazards. Keep a record of the evaluation worksheet for as long as the confined space exists.
