For authorized researchers: Controlled substance logbooks
Authorized faculty or senior research staff members must maintain an up-to-date usage log of their controlled substance, using the SU Controlled Substances Usage Log (Form 3). Keep records locally for at least two years after final disposition of the controlled substance. Logbooks are available for periodic audit by us and the Drug Enforcement Administration (DEA).
Per the Code of Federal Regulations Title 21, Section 1304, keep complete and accurate records of all controlled substances purchased, manufactured, transferred, or wasted.
