How to Use

This form is a tool to help supervisors communicate the hazards, safety procedures, and labeling methods associated with a hazardous substance. Gather information from Safety Data Sheets (SDS) from the manufacturer, standard operating procedures (SOPs), or EH&S‐published fact sheets to complete. Substances with similar properties may be grouped together. Review this form with employees as part of their Local (Tier III) job‐specific Hazard Communication training.
