How to Use
The “On the Job Training Form”, or OJT, provides a record of the protocol specific instruction received by new radioactive material users. Upon completion of the Radiation Safety Training, you will receive a OJT form from Health Physics. You must be instructed on any aspects of their work with radioactive materials which are unique to the laboratory, experimental protocol, or CRA. The individual providing the instruction may be the PI, project contact, or any other authorized radiation worker who is familiar with the hazards and procedures involved. The fully completed OJT form must be filed in the corresponding Radioisotope Journal.
