How to Use

This Spare Dosimeter Assignment Form must be completed when a user is assigned a spare dosimeter. The form shall be returned to Health Physics when the spare is returned and may be emailed to the Health Physics Dosimetry Coordinator (dianagar@stanford.edu) or printed and physically returned to Health Physics with the spare dosimeter. Contact dianagar@stanford.edu or call Health Physics (650) 723-3201 with questions. Please note that spare dosimeters may only be assigned to ONE individual before they are returned.
