How to Use

Incidents, injuries, and illnesses that occur on Stanford University property must be promptly reported to EH&S and the Office of Risk Management. Use this form to report an incident involving a student, contractor, or visitor.

For incidents involving a University employee or student employee, use the SU-17 Incident Investigation Report for Employees.

After completion, submit this form via email to incidentreport@stanford.edu with “SECURE:” in the subject line.
