How to Use

This questionnaire gives us information on control measures in place for toxic gases before purchase. It also lets us ensure compliance with the Santa Clara County Toxic Gas Ordinance (TGO).

Instructions:

Complete this questionnaire before each purchasing a toxic gas.
We will review it and contact you to approve the purchase or request more information.
